§ 20-176.50. Renewal; loss or destruction of certificate or trainee permit; replacement.

(a)

A technician's certification or trainee permit shall be renewed annually upon payment of the required renewal fee.

(b)

In the event of loss, destruction or mutilation of a certificate or permit, the person to whom the certificate or permit was issued may obtain a replacement upon furnishing satisfactory proof of the loss, destruction or mutilation, and upon payment of the appropriate fee. Application for a replacement shall disclose the following information:

(1)

Name and address of the applicant.

(2)

Explanation of the loss, destruction or mutilation of the original certificate or trainee permit.
